Forms and Applications

If you are having difficulty retrieving your tax information through the IRS Data Retrieval Tool or are unable receive your Tax Return Transcript through the IRS; please contact our office at 801-581-6211. You may email us as well at financialaid@utah.edu and please put “Tax Transcript Trouble” in the subject line.

NOTICE: Our office can no longer accept verification worksheets, Parent PLUS loan request forms, and self certification forms over e-mail. Please submit these forms to our office through our online upload tool (below the general section) in person, over fax, or by mail. Contact our office if you have questions.

You can now upload/attach your required documents. Documents that can be uploaded/attached through the links below include: tax documents, W-2(s), verification documents, appeal documents, social security card, driver's license, ect. Chrome and Firefox work best with the uploader.

2021-2022 Verification Forms

The preferred method to submit your tax information is to use the IRS Data Retrieval Tool (DRT) on the FAFSA application. If you are unable to use the DRT please submit your Tax Return Transcript (TRT) to our office via mail, fax or bringing the documents to our office. Please note that personal identifiable information cannot be accepted by the University of Utah via email. This would include, but not be limited to, any tax documents, social security cards, driver’s license, passports, citizenship documents, etc. Please upload, mail, fax or bring the documents into the office

We are happy to announce our group 1 verification form is now electronic. The University of Utah has partnered with DocuSign to provide an e-signature option of the verification form. After the form is submitted the student and parent will each receive an email from DocuSign with a copy of the verification form to sign electronically.
